Sweet Life Diabetes Education Program
MDPrevent created the Sweet Life Diabetes Education Program to provide much needed education, counseling, clinical and emotional support services for our patients with diabetes. Whether you are newly diagnosed or need updated information on living well with diabetes, the Sweet Life Diabetes Education Program will empower you with the tools and skills you need to enjoy a happy and healthy life.
During the Sweet Life Diabetes Education Program, you will work with a team of wellness coaches consisting of Advanced Registered Nurse Practitioner, Certified Diabetes Educators, a Registered Dietitian, and a Licensed Clinical Psychologist. Topics cover will include:
- Glucose targets and how to achieve balance
- How to recognize and prevent complications
- Nutrition and meal planning
- Lifestyle tips which promote good health
- Stress-reduction techniques
As a newly dignosed diabetic, you will experience compassion and support during the Sweet Life Diabetes Program, which consists of a one hour one-to-one counseling session with an Advanced Registered Nurse Practitioner and 2 half-day group sessions. Been living with diabetes for some time? If you were diagnosed with diabetes more than 12 months ago, you are eligible for 3 hours of compassionate and informative nutritional counseling sessions with a registered dietitian each year. We provide a complimentary service for everyone in our community with diabetes – a diabetes support group led by a warm and caring licensed clinical psychologist, Jessica Kordansky, PhD.
Our Diabetes Support Group meets every fourth Thursday from 10-11 am.
